NHSM seeks to come along side parents to help them raise teenagers that love the Lord with all their heart, soul, strength, and mind.  We want to work side by side with them through this stage of life.

Downloadable Forms – These forms are things we use regularly for any event.

ScholarshipForm This form is used if your son or daughter would like to go on a event, but time are hard for your family financially. Fill this out and return it to Nick.

Medical Release Form This form is used whenever we leave the North Haven Church campus. You only need to fill this out once a school year. Once you have filled this out, for every other event you only need to turn in the ‘Basic Permission Form’.

BasicPermissionForm Used only after you have turned in the Medical Release form for that particular school year.
